Hospital Discharge Program
Hospital Discharge Program
Hospital discharge is one of the most vulnerable moments in a patient's recovery. You need transport that's on time, equipped for your condition, and handled by someone who understands post-acute care logistics. That's what we do.
About This Program
Hospitals discharge patients on their schedule — not yours. A discharge can happen with a few hours' notice, on a weekend, or late at night. Our Hospital Discharge Program is built for that reality: we're available 24/7, we work directly with hospital discharge coordinators, and we arrive equipped for whatever the patient needs.
We serve all major Denver-area hospitals including UCHealth University of Colorado Hospital (Anschutz), Denver Health Medical Center, SCL Health Saint Joseph Hospital, Presbyterian/St. Luke's, Rose Medical Center, Swedish Medical Center, Sky Ridge Medical Center, and all Centura and HCA facilities. We transport patients in wheelchairs, those with limited mobility, and patients with medical equipment including oxygen and IV lines.

How It Works
Hospital Calls or You Call
The discharge coordinator can call us directly at 720-210-0058, or you or a family member can call. We coordinate with the floor to confirm discharge timing.
We Confirm the Details
We confirm the patient's mobility needs, any equipment being transported, and the destination — home, SNF, rehab, or assisted living.
On-Time Arrival at the Hospital
We arrive at the correct hospital entrance, coordinate with the floor nurse or discharge coordinator, and assist the patient to the vehicle.
Safe Delivery to Destination
We transport the patient to their destination and assist them inside — not just to the curb. We don't leave until the patient is settled.
